Behavioral Health Specialist 


Community Care is hiring a Behavioral Health Specialist. This is a full-time opportunity.


Behavioral Health Specialist Job Responsibilities:

  • Responsible for providing Behavioral Health (BH) services across Community Care Inc. programs for designated members.

  • Serves as a resource to the Interdisciplinary Team

  • Provides targeted Behavioral Health Case Management services, as assigned.

Behavioral Health Specialist Job Requirements:

Experience: A minimum of 3 years of experience in providing social work and/or behavioral health case management services to older adults and adults who are physically
and developmentally disabled required; Mental health experience, including provision of individual counseling and support group facilitation with older adults is preferred.

Behavioral Health Specialist Education:

Masters Degree, Social Work, Counseling or related field required.

Certifications/Licensures: Wisconsin Licensed Advanced Practice SW required; LCSW or LPC preferred.

*Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.

Behavioral Health Specialist Job Schedule:

  • Monday to Friday - 8:00AM – 4:30PM
