We are currently seeking Service Coordinators in multiple locations across Hudson Valley (Positions available in Putnam, Dutchess, and Orange County.)  

 

 

Description: The Service Coordinator is responsible for assisting in developing, promoting and monitoring a service plan for children and their families.  The work involves assisting families to identify and obtain available and appropriate services as well as conduct intake assessment and referral activities.

 

ESSENTIAL FUNCTIONS:

  • Assists families to identify, develop a plan and obtain services appropriate to their needs.
  • Coordinates and monitors delivery of identified services and the proper use of resources.
  • Advocates for children and their families with service delivery and community agencies and coordinates assessment and evaluation activities.
  • Participates in the development, review and evaluation of individualized family service plans (IFSP).
  • Participates with the family in the IFSP meeting to finalize services that will be provided.
  • Maintains written records and reports regarding service coordination activities.
  • Responds positively to suggestions/directions, maintains a flexible attitude, and generates new ideas or process improvements.
  • Is proactive in maintaining a caseload and providing services as authorized to children and their families. 

  • Communicates problematic issues with respective supervisor.

  • Collaborates with colleagues on at least a monthly basis as evidence by documentation in service notes.

  • Effectively accesses and utilizes Liberty website and employee portal to gain needed information, complete notes, check e-mail, access forms, parent education information, continuing education information, and employee handbook.

  • Attends and participates in continuing education, ongoing staff in-services and quarterly meetings.
  • Follows mandatory guidelines according to the Regional Policies and Procedures and Employee Handbook.

  • Establishes a rapport and effectively communicates and works with related agencies, community members, children, families and co-workers.

    

 Requirements:    

Education: Bachelor’s Degree from an accredited College or University.

Experience: Professional experience providing direct service to children and families is required. 

 

Pay Rates - 

$11/$15 per minute unit monolingual

$12/$15 per minute unit bilingual
